Does low testosterone make you tired: what clinical trials show about fatigue in men

Does low testosterone make you tired? The internet says yes, and plenty of clinics will happily sell you a prescription on that basis. The research tells a more careful story.

Testosterone plays a role in energy, but fatigue alone is a poor sign of low T, and raising testosterone did not reduce fatigue in the largest trial of older men. That matters, because if you blame hormones too quickly you can miss the real cause, which is often easier to address.

What Testosterone Does for Energy

Cleveland Clinic lists boosting energy and mood among testosterone's jobs, along with building muscle, keeping bones strong, supporting sex drive and helping make enough red blood cells. So a link with tiredness is plausible.

Levels also fall naturally. Cleveland Clinic says they can drop about 1 percent a year from your late 30s, and it notes that providers consider levels below 300 ng/dL low in adults. The diagnosis of male hypogonadism applies when you have symptoms as well as low numbers. A number alone does not explain how you feel.

What the Testosterone Trials Found

The strongest evidence comes from the Testosterone Trials, published in the New England Journal of Medicine in 2016. Researchers gave 790 men aged 65 or older, all with testosterone under 275 ng/dL and symptoms, either testosterone gel or placebo gel for one year.

Levels rose to the mid-normal range for men aged 19 to 40. The results split sharply:

  • Sexual function improved. Sexual activity, desire and erectile function all rose significantly.
  • Mood improved slightly. Men reported better mood and fewer depressive symptoms.
  • Fatigue did not. There was no significant benefit for vitality, measured on a standard fatigue scale.

In other words, these men corrected their hormone levels and were no less tired than the placebo group after a year.

Keep the limits in mind. The study covered only men 65 and older with moderately low levels. It does not settle the question for a 35-year-old, or for a man whose hypogonadism has a clear medical cause. The authors also said the trial had too few participants to draw conclusions on the risks of treatment.

Why You Might Be Tired With Low T Anyway

Low testosterone and fatigue often show up together because they share causes. That is different from one causing the other.

Sleep apnea. Cleveland Clinic lists obstructive sleep apnea as a condition linked to low testosterone. The NHS separately lists it as a cause of daytime tiredness, particularly with loud snoring or waking up choking. One condition can give you both complaints.

Excess weight and type 2 diabetes. Cleveland Clinic names obesity and poorly managed type 2 diabetes as factors that make low testosterone more likely. The NHS lists diabetes as a cause of tiredness too.

Alcohol and medications. Alcohol use disorder, opiates, anabolic steroids and certain other medicines appear on Cleveland Clinic's list of causes.

Depression. The NHS includes stress and depression among common causes of fatigue. Since low mood can overlap with low T symptoms, it is worth raising both with a doctor.

Other Causes to Rule Out

The NHS lists several more likely explanations when tiredness will not lift:

  • Too little sleep or insomnia
  • Poor diet and not enough exercise
  • Iron deficiency anaemia
  • Thyroid problems
  • Recent infections such as glandular fever or COVID-19

If you cannot explain your tiredness and it is not improving, see a GP rather than self-diagnosing. A blood test can check several of these at once.

How to Tell If Low T Is the Likely Culprit

Fatigue on its own points in many directions. It becomes more suggestive when it comes with sexual symptoms such as lower desire or weaker erections. Our article on how to tell if you have low testosterone walks through the pattern.

Ask your doctor for a blood test. Cleveland Clinic advises speaking to a provider if you have signs of low testosterone, and treatment decisions should rest on your symptoms plus your lab results. If testosterone therapy comes up, ask what specific symptom it is expected to change. Based on the trials above, sex drive is a more realistic target than energy.

What You Can Do Now

Cleveland Clinic's lifestyle advice for lowering the risk of low testosterone also happens to cover most causes of tiredness:

  1. Sleep seven hours a night, and treat sleep apnea if you have it.
  2. Stay physically active. Regular movement supports both weight and mood.
  3. Keep to a healthy weight and eat well.
  4. Limit alcohol and other addictive substances.

Training your pelvic floor will not raise your energy, and we will not claim it does. But if low testosterone has come with weaker erections, it is a useful habit that sits alongside the rest of this list, because the muscles involved in rigidity are ones you can train. Our guide to how erections work shows where those muscles fit in.

The Bottom Line

Low testosterone can contribute to tiredness, but do not assume it is the cause. In the best trial we have, raising it improved sex life and mood but not fatigue. Get your levels tested, check for sleep apnea and the other common causes, and work on sleep and activity first.

Build one habit that sticks

Defy keeps pelvic floor training simple: sessions from three minutes a day, a streak to protect and progress you can track.

Frequently Asked Questions

Does low testosterone make you tired?

It can be part of the picture, since testosterone supports energy, mood and red blood cell production. But the link is weaker than most men assume. In the Testosterone Trials, raising testosterone for a year in older men with low levels gave no significant benefit for fatigue.

Will testosterone therapy give me more energy?

Not reliably. The Vitality Trial, part of the 2016 Testosterone Trials in the New England Journal of Medicine, found no significant benefit for vitality, though men reported slightly better mood and fewer depressive symptoms. Those men were 65 or older, so results may differ in younger men or those with clear medical causes of hypogonadism.

What testosterone level counts as low?

Cleveland Clinic says providers consider levels below 300 ng/dL low in adults, and they call it male hypogonadism when symptoms are present along with those low levels.

What else could be making me tired?

The NHS lists poor sleep, stress, depression, unhealthy diet and lack of exercise as common causes, along with medical conditions such as sleep apnoea, iron deficiency anaemia, diabetes and thyroid problems. A GP can check for these.

Does sleep apnea lower testosterone?

Cleveland Clinic lists obstructive sleep apnea among the conditions linked to low testosterone, and untreated sleep apnea also causes daytime tiredness on its own. If you snore loudly or wake up gasping, get checked, since this is a treatable cause that can sit behind both problems.
