Over the counter ED meds: what is FDA-cleared versus unregulated supplements

Most articles about over the counter ED meds are either supplement ads or blanket warnings that nothing sold without a prescription works. Both are out of date. Since 2023 there is exactly one nonprescription ED treatment cleared by the FDA in the US, and in the UK you can buy real sildenafil at a pharmacy counter.

Knowing precisely where that line falls is the difference between a legitimate option and a capsule with an unlabelled dose of prescription drug in it. Here is the current picture.

The One Legitimate OTC Option in the US

In June 2023 the FDA granted marketing clearance to Eroxon (MED3000), a topical gel, making it the first over-the-counter erectile dysfunction treatment available in the US without a prescription. Harvard Health covered the clearance when it happened.

Two honest caveats. The speed of onset is genuinely faster than an oral tablet. But the evidence base is thin next to the decades of randomized data behind PDE5 inhibitors, and clinicians have flagged the same concern that applies to every OTC route — it lets men skip the diagnostic step entirely.

The UK Is Ahead on This

Why Skipping the Doctor Costs More Than It Saves

The appeal of buying off a shelf is obvious. No appointment, no conversation about your erections with a stranger holding a clipboard.

But erectile difficulty is frequently the earliest visible sign of something systemic — narrowing blood vessels, low testosterone, poorly controlled blood sugar, or a side effect of medication you already take.
